At Health First, medical records are retained for a minimum of 20 years. The records are retained in various formats including film, microfiche, paper, and electronic. The Health Information Management (HIM) Department collects, manages, protects, and disseminates the health information of our patients, while ensuring the information is complete and accurate.

You have the right to:

• Request copies of your medical records - for you or your physician
• Request an amendment to your medical record - if you believe the information is not correct
• Request a list of disclosures of your medical information - to receive a list of parties to which your medical information has been disclosed
